As product designer, I mapped what data mattered to whom, claims for payers, health trends for members, prescriptions for pharmacies, and consolidated five fragmented sources into one continuously updating view.

I built a two-part scoring system that made passive health data visible and actionable, then used personalisation to turn into specific next steps: nudges, and alerts tied to what was actually moving each member's health-score.
